- Your completed Patient Information form if you have not already submitted it online.

Referrals
A referral is required for all medical and cosmetic consultations and must be brought to the appointment on the day of your consultation or sent to us prior to the day.
It is now an AHPRA requirement that all cosmetic patients bring a valid referral from their GP specifying the reason for a consultation with Dr Safvat. Legally, Dr Safvat will be unable to see the patient if the referral is not available at the time of the appointment.
The Consultation
During the consultation, Ethique Plastic Surgery’s Dr Andre Safvat, will give you detailed information relevant to your needs and desired outcome. Dr Safvat will discuss the following (amongst other things):
- Options for treatment
- The procedure
- Any risks
- The expected outcome
- Recovery expectations
- An indication of whether your procedure is medically required – Medicare/Health Fund rebates may be available if the procedure is classified as medically required.
- Answer your questions
Following the appointment, you will be given a thorough estimate of fees for any potential procedure, including rebate estimates from Medicare and Private Health Funds (where applicable), so you know exactly what you are up for in terms of costs. There is absolutely no obligation to go ahead with any sort of procedure after having a consultation and receiving an estimate.
